What is a Facelift?

A facelift, or rhytidectomy, is a surgical procedure that removes excess facial skin and lifts the deeper tissues (SMAS) to create a more youthful appearance and minimize visible skin wrinkles and jowl lines on the face and neck. The results can be subtle or dramatic, depending on the effects the patient wants to achieve.

Aging of the face is inevitable. As the years go by, the skin on the face and neck begins to loosen. Crow’s feet appear at the corners of the eyes. Fine forehead lines become creases, then gradually deepen into deeper folds. The jawline softens into jowls, and beneath the chin, another chin or vertical folds appear at the front of the neck. Heredity, personal habits, the pull of gravity, and sun exposure all contribute to facial aging. As the aging population grows, it is understandable why rhytidectomy has become the third most popular facial plastic surgical procedure.

Understanding the Procedure

Many names are used for the mini facelift; all refer to a less invasive facelift surgery than the traditional facelift. When performing a mini facelift, Dr. Moradzadeh uses shorter incisions that promote faster healing, reduce swelling and bruising, and lead to a faster recovery than a traditional facelift. The shorter incisions are also easier to hide, and allow patients to wear pulled-back hairstyles that expose the backs of the ears.

Shorter incisions are possible because the manipulations during a mini facelift are less extensive than those during a traditional facelift. The facial skin is pulled just slightly, and skin excision can be relatively minor, resulting in a totally natural-looking appearance without the “pulled face” look.

What to Expect Afterwards

Depending on the surgical approach and any accompanying plastic surgery procedures, the mini facelift can be completed in just a few hours. Dr. Moradzadeh uses local anesthesia with variable degrees of sedation for most mini facelift procedures. Patients are awake but do not experience any sensation during the procedure. Most mini facelift patients can return to light activities and work within 1 week, and to strenuous activities and exercise within 2 weeks. During recovery, most patients do not experience significant pain or major discomfort, and unpleasant sensations can be managed with appropriate pain medications.

Is a Mini Facelift Right For You?

Typically, mini facelifts are performed on men and women in their 40s and 50s — individuals who desire a slight rejuvenating effect, not a dramatic makeover. They might have some jowls, minor skin laxity along the jaw line, and/or wrinkles in the lower face. The mini facelift procedure can address all of these aesthetic concerns. However, a mini facelift is not beneficial for patients with a large amount of excess, droopy skin. For these individuals, the SMAS or deep plane facelift is a better option.

What’s the First Step?

What to Expect on the Day of Facelift Surgery

A facelift is a surgical rejuvenation of all four of the following areas: the forehead and upper eyelids, the midface and lower eyelids, the lower face and lips and the neck.

Facelift surgery continues to evolve. Over the years, the procedure has evolved from simple skin-lifting to more complex, deeper-plane lifting techniques. The treatment usually involves incisions made in front of and behind the ear, often extending into the scalp. Once the incisions are made, various degrees of skin manipulation are performed, and the deeper layers of the face are ‘lifted’. Muscle tightening and neck and jowl liposuction may also be performed, with excess skin removed or repositioned. The results are a reversal of gravity’s effects and a tightening of the soft tissues of the face, restoring a more youthful contour.

After trimming excess skin, the surgeon closes the incisions with fine sutures and/or metal clips, allowing the surgery to proceed without shaving the incision site. Depending on the extent of the surgery, the procedure can take anywhere from 2 to 4 hours. When the procedure is performed with a combination of mild sedatives, local anesthesia, and a mild intravenous anesthesia, the patient will experience little discomfort. Following surgery, the surgeon will apply a dressing to protect the entire area where the incisions were made.

Facelift Recovery

Your Guide to Post-Facelift Care

The recovery time after facelift surgery varies among individuals. Elevating the head is extremely helpful in reducing initial postoperative swelling. Intermittent use of ice packs may also help relieve the swelling. Patients should avoid aspirin and non-steroidal anti-inflammatory medications for the first few days after surgery. Smoking and exposure to second-hand smoke should also be avoided to prevent delays in the healing process.

Once the dressings are removed, puffiness and discoloration may be more pronounced in some portions of the face than others. Patients needn’t be alarmed by any unevenness or temporary asymmetry that appears in the face. This is normal. While most swelling occurs within the first 24 to 48 hours, mild swelling, bruising, and numbness may persist for several weeks. Continuing to sleep upright for several nights after the operation will also reduce the swelling.

In addition to swelling, patients may experience numbness in the face and neck for several weeks after surgery—sometimes longer. It is important to avoid any bending or heavy lifting for two weeks following surgery because this can cause blood to rush to the face, leading to discomfort and occasionally some minor bleeding. The sutures around the ear are usually removed between 5 and 7 days after.

On the third or fourth day after the procedure, patients can generally start washing and gently blow-drying their hair. Cosmetic make-up may be used, but should not be applied around the suture line to allow the suture line to heal for 2 to 3 weeks after surgery. Perming and hair coloring can resume four to six weeks after surgery, depending on the extent of the surgery. As with any major surgery, patients may feel tired for a few days after the operation, and this fatigue can last for 2 to 3 weeks.

Serious physical activity, including aerobics, should be avoided for up to 1 month after surgery, although gentle exercise, such as swimming, is permitted once the wounds are fully healed. Straining, bending, and lifting should be avoided during the early postoperative period, as these activities may increase the risk of bleeding.

Dr. Moradzadeh will provide specific post-operative instructions, including how to care for dressings and incisions, which medications to take, what to watch for, and when to schedule an office visit to monitor healing. It may take several months for the swelling to fully resolve, and up to 6 months for the incision scars to fade fully. Avoiding factors that could reduce the benefits of surgery, such as excessive unprotected sun exposure and smoking, is essential.
